O.K. Here are the documents promised. The files are posted on our

Here are a few points for discussion. You will notice that the
Articles of Incorporation require that you incorporate in one State
and are governed by the laws of the incorporating State. We are
attaching a draft copy of the Articles of Incorporation for the state
of Ohio (our home state) for your reference. This is pending
approval for the group.

Rich and I volunteer to accept leadership roles in this
organization. We have already spoken to several of our friends and
contacts and there is an interest from them in serving this
organization as volunteers. We also have a number of relatives very
active in philanthropy and will provide us with guidance and support.

The enclosed By-laws are for your review. We have created a first
draft using the name Fibromuscular Dysplasia Society of America
(FMDSA). This is not set in stone. We also stipulated that we have
a voting membership that will elect the Board of Directors and
approve all changes to our Articles of Incorporation and By-Laws. As
a note, members are not required to attend the annual meetings to
cast their vote. The objective here is to operate as democratically
as possible and to ensure the long term health of the organization.

We have also enclosed documents defining NORD's policies and
programs. NORD is a vital cog in our research efforts. As such, we
do recommend becoming an Associate member of NORD once we have met
their membership requirements.

Lastly, we feel it is important for all of us to take an active role
in the organization. Rich and I volunteer to become the initial
incorporators of the organization. As you will see from the
Organizational Plan and By-laws, there are many positions to be
filled and a Board of Directors must be elected as well (sometime
within the first year). We have enclosed a brief bio of ourselves
and we ask that everyone else do the same.
